Ready for your child’s visit?

Remember to bring the following to your child’s first visit. If it is not the first visit, make sure you let the clinic know if any of the following information has changed:

  • Name and address of previous doctor to get records
  • Insurance information
  • Vaccination records
  • Medication lists or bottles
  • Date of last well child visit

About Well-Child Exams

When you should come in for a well-child visit depends on your child’s age. We follow guidelines set by the American Academy of Pediatrics. Well-child visits are recommended at the following ages for infants and toddlers:

  • First-week visit (3-5 days old)
  • 2 weeks old
  • 1 month old
  • 2 months old
  • 4 months old
  • 6 months old
  • 9 months old
  • 12 months old
  • 15 months old
  • 18 months old
  • 2 years old (24 months)
  • 2 ½ years old (30 months)
  • Children and young adults age 3 to 21 should come in for a visit once a year.

Is there a cost for well-child exams?

There is no cost for a well-child exam for those with most insurances. including Medicaid. Please show your insurance information or CARE Program information at your visit. If your child receives more services than a well-child exam at their visit, you may need to pay a deductible or copay.
